Move the index generation step up one LaTeX render; LaTeX needs to
render the table of contents or the index pages end up in the wrong
place. LaTeX only renders tables of contents on the second run. Then we
need a third for the Index to be added to the file.

Must be run as somebody with permissions to write temp files and 
access /home/httpd/html/adm/help/eps.
