Fixed a bug that caused the display of 2 breadcrumblines (1 of which erroneous) if course contents was the first resource in course.

Box title is now the main title of the next page and the link text or the former page title will be set to the subtitle in the near future.
Group: consistent wording

- "Re-initialize" button in roles screen no longer restricted to advanced users.
- Show appropriate message on roles screen when navmap object could not be created because inactivity caused deletion of $user_$course.db and/or $user_$course_params.db from lonDaemns/tmp directory.

Localization:
Added some missing mt() calls and optimized mt() call

Change arguments accepted by lonnet::appenv() to:
1. reference to hash
2. (optional) reference to array.

Change all instances where lonnet::appenv() is called to replace first argument passed (originally a hash) to a reference to the hash.

- Modify lonnet.pm rev 1.35 code intended to prevent "dangerous" modifications of the environment, so it will actually do this.
- Allow these modifications to be made in certain instances:
   DC switching to adhoc role
   CC switching to a different course role in the current course.
   user self-enrolling as a student

- bug#4718, problems with no due date set were mucking with the  show first due homework

- splitting the handlery bits of lonnavmaps from the functional components
