File size checking for uploaded files

A new javascript file, file_upload.js, handles client side file size checking.
The function, checkUploadSize(), accepts an html file input element and a maximum
file size (in bytes) as input.  If the size of the file to be uploaded exceeds the
size allowed, a message is displayed the the element is cleared.  Server side
checking is still used if it passes this test.

Loading file_upload.js adds event handlers to file input elements with
class="flUpload".

It also looks for a hidden element with id="free_space" that contains the maximum
upload size.

The free space is usually calculated by subtracting a user's disk usage from their
quota allowance.  In some cases it is a fixed value.

I have implemented this for every upload instance I could find.  These include:

* Authoring space
* Portfolio

* Helpdesk request forms
* RSS feed uploads

Users with "mpq" privilege for a domain can set and modify the portfolio quota for a user in the domain.

When a user uploads a file to his/her portfolio, the user's portfolio quota is retrieved, if one has not been set, the default for the domain is used, if no default has been set, the original static value of 20 Mb for backwards compatibility.

In the future we might want to consider different defaults for different types of user in a single domain.

Selection of roles, access types, sections and groups for each entry in a course-based condition for access to a portfolio file now occurs in a separate window.  Display of select boxes occurs immediately for new entries for existing course-based conditions, but after course selection via lonpickcourse for new course-based conditions.  Results of choices are stored in hidden form elements in opener window.

Advertising a user's viewable portfolio files on the user's aboutme page. Currently not implemented for group portfolio files. File sizes for uploads to course group portfolios may not exceed quota set for group's file repository. When creating a course, a quota can be specified.  The sum of quotas assigned to all groups in a course can not exceed the total quota defined for the course (when the course was created).  - client side and server side enforcement still needed here.  Portfolio permissions modified to allow any user with group creation rights in a course access to all course group portfolio functions.  All uses of words: group, Group, course and Course in "Course Groups Manager" now replaced with context-sensitive terminology provided by loncommon::course_type() and loncommon::group_term() which will supply appropriate text once a decision is made on Group vs Group. Currently "ANGEL-like Discussion Groups" are called Groups, regular courses are called Courses; and within regular courses sectionesque units are called groups, and within Groups they are called teams.

Additional portfolio access control can now be defined: passphrase-protected, or conditional access (requires log-in) based on username/domain, domain, course or group affiliation (possible restriction by role, status, sectio and group).

Enable users to set access controls on portfolio files to permit public viewing. Third iteration of data structure in file_permissions.db for storage of access contol records. Now uses key = value for individual access control records and a key = anonymous hash for fast look-ups of all records for a single file.   Use of a temporary lock record to prevent others modifying access control records for a specific portfolio file while the record for the same file is being updated. Additional argument added to call to lonhtmlcommon::datesetter() to optionally suppress display of calendar link.

file_permissions.db now stores information for each file as an array of arrays and a hash. In the hash, keys can either be accesscount or access. For key = access value is an anonymous hash containing key=value pairs defining access controls. For key = accesscount, value is a scalar (integer to be used when creating the next accesscontrol). Documentation added.

Work in progress.  Adding access control entry to file_permissions.db.  Current implementation is as additional arrays in array of arrays for each key (key =f file name - including path). New arrays contain either 'access' or 'accesscount' as first element.  These will become hashes in the next iteration. Added column to portfolio directory displays to show access status.  Some fixes to paths to handle coursegroup portfolios - e.g., new &prepend_group() function.

&coursegroups() and &get_group_settings() moved to longroup.pm, which contains general utility functions for asking about groups.  Also contains &group_changes() which is used to add/drop group memberships as a result of role changes, as determined by group settings for auto-add and auto-drop. 'none' and 'all' are now reserved words which may not be used as section or group names, so they can be used instead of _all and _none  when specifying auto-add and auto-drop settings for all sections or no section roles.

Portfolio concept extended to include upload of files to a "portfolio" area within a group in a course to facilitate sharing of files in small instructor assigned teams in a course.  If this ends up being used, work on lonmeta will be needed, and control of who can upload/delete etc will need to be enforced using user's prvileges in the specific group.

	Still very much under construction. Beginning of directory
	navigation is working. Upload seems to work, but needs to be
	made more secure.
